SGLT2 Inhibition in Acute Myocardial Infarction—A Comprehensive Review

چکیده

In heart failure as well in chronic kidney disease sodium-glucose cotransporter 2 (SGLT2) inhibitors have changed the landscape of medical therapy. Originally developed for use diabetes, an unforeseen cardiovascular benefit extended SGLT2 inhibitor from antihyperglycemic agents to and renal risk modifying agents. As their is independent diabetic state left ventricular ejection fraction it only class therapy recommended throughout spectrum failure. Until very recently, remaining gap evidence has been data on safety efficacy patients with acute myocardial infarction (MI) former trials date excluded recent ischemic events. first out three conducted post MI EMMY trial was published. randomized 476 shortly after percutaneous intervention large either 10 mg empagliflozin daily or placebo. The primary endpoint changes N-terminal pro brain natriuretic peptide (NT-proBNP) over 26 weeks functional structural secondary endpoints were met. This provides inhibitors-mediated beneficial results this group patients. We here discuss these light two upcoming outcome (DAPA-MI EMPACT-MI) regard future role drugs early MI.

Reperfusion strategies in acute ST-segment elevation myocardial infarction: a comprehensive review of contemporary management options.

There are an estimated 500,000 ST-segment elevation myocardial infarction (STEMI) events in the U.S. annually. Despite improvements in care, up to one-third of patients presenting with STEMI within 12 h of symptom onset still receive no reperfusion therapy acutely. Clinical studies indicate that speed of reperfusion after infarct onset may be more important than whether pharmacologic or mechani...

Acute tumor lysis syndrome: a comprehensive review.

Tumor lysis syndrome is characterized by the massive destruction of malignant cells and the release in the extra-cellular space of their content. While Tumor lysis syndrome may occur spontaneously before treatment, it usually develops shortly after the initiation of cytotoxic chemotherapy. These metabolites can overwhelm the homeostatic mechanisms with development of hyperuricaemia, hyperkalaem...

SPOTLIGHT REVIEW Proteasome inhibition during myocardial infarction

The ubiquitin-proteasome system (UPS) plays a central role in protein degradation and regulates a variety of critical cellular processes. During recent years, the cardiac UPS has become increasingly recognized as a key regulator of cardiac function under both physiological and pathological conditions. Numerous studies have demonstrated that altered UPS function is involved in the pathogenesis o...

Apple Trees to Sodium Glucose Co-Transporter Inhibitors: A Review of SGLT2 Inhibition

For centuries, the kidney has been considered primarily an organ of elimination and a regulator of salt and ion balance. Although it was once erroneously thought to be the structural cause of diabetes and in later years was ignored as a regulator of glucose homeostasis, it is now recognized as an important player in the arena of glucose regulation. Today, we have a better understanding of the p...

Renin-Angiotensin System Inhibition in Acute Myocardial Infarction in Dogs

Acute left anterior descending coronary artery occlusion was produced in 21 conscious, chronically instrumented dogs. Forty minutes after coronary occlusion, nine dogs were given i.v. teprotide, 25 ,ug/kg/min, followed by oral doses of captopril, 10 mg/kg every 8 hours for 24 hours. The remaining 12 dogs served as saline-infused controls. In all dogs, acute coronary occlusion increased plasma r...
